There is currently a national shortage of medications to treat ADHD symptoms. This is due to manufacturing issues and the growing demand for ADHD medications worldwide.

Our service is able to provide private ADHD titration as an alternative to waiting for titration NHS funding through Right to Choose. Our assessment fees cover the cost of one prescription.

Titration

A thorough evaluation of your ADHD symptoms will be carried out by a highly experienced consultant Psychiatrist. This will include an examination of your medical history as well as an assessment using the standard ADHD rating scales and questionnaires.

After your examination After your assessment, you will be informed whether medication is appropriate for your situation. This will be discussed during your follow-up appointment (which can take place in person, via video link, or over the phone). Your physician will go over the different options for medication available. If you decide to start medication, we will help you the opportunity to increase your dosage to optimize the benefits/side-effects balance for you.

This is a process which can take several weeks and involves adjusting the dosage of your child's medication slowly until the right balance is reached. It's not uncommon to have to titrate as high as three different medications before you find the one that is most suitable for your child's ADHD. It's important for parents to participate in this titration process and keep a close watchful eye on their child's behavior, so that they can inform the doctor whether the medication is working or causing side effects.

If you decide to go back to NHS treatment, you will receive a private prescription for your medication. You will have to pay for your prescription at the pharmacy. This is not included in the consultation fee. You will need to attend six-monthly appointments to monitor your medications. This includes blood pressure and pulse measurements. Annual exams are also required.

Follow-up appointments

Once the assessment has been completed you will be able to make follow-up appointments with the psychiatrist as necessary. The appointment usually takes place via a zoom video call but in certain circumstances it might be possible to do an audio-only consultation (again at an extra cost).

In this session, we'll discuss any treatment options we have and titration come up with a unique ADHD medication titration plan. This will include a discussion and review of your medication dose, as well as any side effects that you may be experiencing. The cost of these titration sessions is PS150 for face-to-face sessions and PS60 for online or telephone video consultations. The pharmacy will be charged for any cost of medication.

If you want to start treatment immediately you can start treatment when you are diagnosed. You can also wait two weeks to consider the implications of the diagnosis and what options you have.

We suggest that if are unsure of your preferences, you speak to your GP prior to booking your private examination. Certain providers have different policies on whether or not they require an GP letter. We also recommend to check if your GP accepts a "Shared-Care Agreement', which means that once you are discharged from the clinic, you will be returned to them for the prescription for the medication.

Medication

Medication is a key element of the process for many people looking for a private ADHD diagnosis. It isn't easy to obtain an appointment with a psychiatrist on the NHS. NHS waiting lists are notoriously long, and GPs may be reluctant to refer patients on to specialist services.

One way to get around this is to ask your GP to refer you through Right to Choose (see psychiatry-uk and ADHD 360). This means that you can select a provider that offers a private assessment and titration. This will cut down your waiting time considerably and can be more convenient if you're working and have a full schedule.

Once you've been diagnosed, your doctor will examine you to determine if require medication titration. You'll have to fill out forms that vary by doctor, but the most common ones are self-reporting questionnaires and medical histories dating to childhood and adulthood. These forms will help your clinician determine if there are circumstances that indicate that you shouldn't be taking ADHD medication, even if they have been prescribed.

If you are on a titration medication, this will usually include a mix of non-stimulant and stimulant medications. However, for those who do need further discussions about their titration and treatment plan it's possible to be referred back to the private clinic that made their initial assessment. If this happens, the titration appointment will be part of their consultation fee, and they will be given their medication on an individual prescription (see Consultation fees above). It could cost more than an NHS prescription since the pharmacy will charge for the medication.
