Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that supports research on pragmatic trials. It shares clean trial data and ratings using PRECIS-2 allowing for multiple and diverse meta-epidemiological studies to compare treatment effects estimates across trials with different levels of pragmatism and other design features.
Background
Pragmatic trials are becoming more widely acknowledged as providing evidence from the real world for clinical decision making. Despite these requirements, many RCTs with features that challenge the notion of pragmatism were incorrectly labeled pragmatic and published in journals of all types. This could lead to false claims of pragmatism, and the use of the term should be standardised. Methods

) colleagues10 created an adaptation of the assessment, dubbed the Pragmascope, that was easier to use for systematic reviews. They found that pragmatic reviews scored higher in most domains, but scored lower in the primary analysis domain.
This difference in primary analysis domain can be due to the way in which most pragmatic trials analyse data. Certain explanatory trials however do not. The overall score for systematic reviews that were pragmatic was lower when the areas of organization, flexible delivery, and following-up were combined.
It is crucial to keep in mind that a pragmatic study does not necessarily mean a low-quality study. In fact, there are increasing numbers of clinical trials which use the term "pragmatic" either in their title or abstract (as defined by MEDLINE but which is not precise nor sensitive). These terms could indicate that there is a greater understanding of pragmatism in abstracts and titles, however it's unclear whether this is evident in content.
Conclusions
In recent years, pragmatic trials have been becoming more popular in research as the importance of real-world evidence is becoming increasingly acknowledged. They are clinical trials randomized that evaluate real-world alternatives to care instead of experimental treatments in development, they involve patient populations which are more closely resembling the patients who receive routine care, they employ comparators that are used in routine practice (e.g. existing drugs) and depend on the self-reporting of participants about outcomes. This method is able to overcome the limitations of observational research like the biases that come with the reliance on volunteers and the limited availability and codes that vary in national registers.
Pragmatic trials offer other advantages, including the ability to leverage existing data sources and a higher probability of detecting meaningful differences from traditional trials. However, these tests could have some limitations that limit their validity and generalizability. For instance, participation rates in some trials may be lower than anticipated due to the healthy-volunteer effect and incentives to pay or compete for participants from other research studies (e.g., industry trials). The requirement to recruit participants in a timely fashion also restricts the sample size and impact of many pragmatic trials. Practical trials aren't always equipped with controls to ensure that the observed variations aren't due to biases in the trial.
The authors of the Pragmatic Free Trial Meta identified 48 RCTs self-labeled as pragmatist and published from 2022. They assessed pragmatism using the PRECIS-2 tool that includes the eligibility criteria for domains as well as recruitment, flexibility in adherence to intervention and follow-up. They found 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or above) in at least one of these domains.
Trials that have a high pragmatism score tend to have broader eligibility criteria than traditional RCTs, which include very specific criteria that are not likely to be used in the clinical setting, and contain patients from a broad variety of hospitals.
